F. Miranda notes that the surname is also linked to the Latin forms Coccus and Cochus. Its transformation into a hereditary surname is documented as early as the 12th century in Lombardy, where a Johannes Cocco appears in a medieval carta venditionis preserved in the Codex Diplomaticus of Milan.

Miranda also suggests that in some cases the surname may have acquired an affectionate meaning similar to the Italian word cocco, meaning "beloved" or "pampered person." However, he considers the occupational origin to be the most probable, especially for southern Italian families.

V. Blunda rejects the affectionate interpretation for Sicily and argues that Sicilian families bearing the surname derive exclusively from the dialectal term còcu, meaning "cook." According to Blunda, the surname reflects the social importance of this profession within the traditional institutions of the island.
