De Felice considers forms such as Benincà to be secondary graphic variants that developed later, especially in northern Italy.
E. Rossoni likewise traces the surname to the medieval personal name Benincasa and records early documentary evidence, including the mention de montibus Benincasa in Cremona in 1092 and the name of Abbot Benencasa Dei in Solofra in 1178. He also suggests that, in some cases, a toponymic origin cannot be excluded. Rossoni regards Benincà as a later alteration of the original surname.
The surname achieved particular historical significance through Saint Catherine of Siena (1347–1380), born Caterina Benincasa, one of the most influential figures in Italian and European religious history. Her prominence contributed to preserving the surname in historical memory.
Today, Benincasa and its variants are found in several Italian regions, with notable concentrations in Calabria, Campania, Puglia, Tuscany, Emilia-Romagna, and Lombardy.
