The authors agree that the surname has an anthroponymic base and goes back to the name of Germanic origin Frank. F. Miranda specifies that the Germanic etymon is the adjective franka, meaning "valiant, bold, free", and that the surname reflects a double condition: ethnic, linked to belonging to the people of the Franks, and political-social, referring to those who enjoyed full rights in the territories under Frankish dominion. Surnames such as Franco, Franchina, Franchetti and Di Franco share this same root.

It is considered that the surname could also be a matronymic. Miranda notes that its main diffusion is concentrated in Sicily, with strong presence in the Messina area —including localities such as Galati Mamertino, Tortorici and Capo d'Orlando—, and notable presence in the Catania, Palermo, Syracuse and Trapani areas, as well as the area of Enna. Surnames in Italy mentions the Val Seriana as a zone of presence. Miranda likewise indicates that the surname is documented in Lombardy, Piedmont, Lazio and other Italian regions.
