The authors agree that this surname derives from the medieval name Alaimo, in turn from the Germanic Heimo or Haimo, possibly an abbreviated form of compound names with the root heim, a Germanic word meaning "house" or "homeland", such as Heimeran or Heimeric.

E. Rossoni notes that in some cases the surname may also derive from the place name Alaimo di Lentini, and documents the lineage in the 13th century with count Alaimo di Lentini, Grand Justiciar of the Kingdom and governor of Messina.

F. Miranda indicates that it is a typically Sicilian surname, with presence also in Lombardy, Piedmont, Lazio, Tuscany, Emilia-Romagna and Veneto. In Sicily it is spread mainly in the provinces of Agrigento, Palermo, Caltanissetta, Enna, Catania and Syracuse.
